During a new patient or regular consult, the doctor may identify that you meet strict Medicare requirements that make you eligible to receive a care plan. During a care plan consult, the doctor & nurse will create a plan to enable the following: Identify health goals You want to achieve Set your own health goals Link you to appropriate allied health support Nurse calls & consults to see how you're going The ultimate aim is to provide you with care that enables you to frequent the doctor less, so that you can enjoy the beautiful place in which we live more. |
Routine & Emergency Procedures
Includes skin surgeries (including those identified during skin clinic), flaps, laceration repairs, toenail removal, ingrown toenail, removal of foreign bodies (including those found in eyes), abscess drainage, aspiration of haematoma.

Home Visits
There are a select few patients who are visited by their doctor at home. If you feel you are unable to attend in person, please speak to the Shore Care Team so that we can see if you may be eligible to receive this service.

Palliative Care is offered to all patients in our community.
Dr Jackson and the Shore Care Team maintain close communication with
the palliative care teams of
Bundaberg, Gladstone & Sunshine Coast University Hospitals
so that patients have the opportunity
to be cared for in the comfort of their own homes.
